NO SURPRISES ACT/Good Faith Estimate

You have the right to receive an estimate explaining how much your health care will cost. Under the law, health care providers need to give patients who don’t have insurance or who are not using insurance an estimate of the bill for medical items and services.

  • You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services. This includes related costs like  medical tests, prescription drugs, equipment, and hospital fees.

  • Make sure your health care provider gives you a Good Faith Estimate in writing at least 1 business day before your medical service or item. You can also ask your health care provider, and any other provider you choose, for a Good Faith Estimate before you schedule an item or service.

  • If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill.

  • Make sure to save a copy or picture of your Good Faith Estimate.

Therapy Rates

Individual therapy – 60 minutes $185

1-hour sessions are needed for Cognitive Processing Therapy; 90-120 minutes are needed for Prolonged Exposure Therapy which is billed at the $185 per hour fee prorated

Individual therapy – 45-50 minutes $175

Most sessions will be scheduled for 45-50 minutes on a weekly basis.

Fees are due at the end of each session.

Professional Services Fees

$100 per hour prorated

This fee is prorated for the following types of services: completion of forms or preparation of letters requested by the client, telephone calls longer than 10 minutes with client, preparation of treatment summaries, telephone calls with other treating providers or contacts affiliated with the client and their care, and time spent performing other miscellaneous services requested by the client.

Career Assessment Package

$500-$600

See the Career Assessment section at the Services link of this website for a description of what is included in this package. The lower fee includes just one feedback/planning session and the higher fee incorporates an extra feedback/planning session if so desired. 1/2 of the fee is due at the end of the intake interview session and the remaining 1/2 of the fee is due at the beginning of the final feedback session.

NOTE: A FREE PHONE CONSULTATION WITH THE POTENTIAL EXAMINEE’S ATTORNEY MUST TAKE PLACE BEFORE AN APPOINTMENT WILL BE SCHEDULED IN ORDER TO CONFIRM THAT I AM AN APPROPRIATE PROVIDER FOR THE CASE. DURING THIS PHONE CALL, I WILL BE HAPPY TO PROVIDE AN ESTIMATED COST FOR THE EVALUATION BASED UPON INFORMATION PROVIDED BY THE ATTORNEY ABOUT THE CASE. ACTUAL COST, HOWEVER, MAY EXCEED THE ESTIMATE BECAUSE IT IS VERY DIFFICULT TO KNOW EXACTLY HOW LONG AN INTERVIEW OR TESTING PROCESS WILL TAKE – PARTICULARLY WHEN INTERPRETERS ARE INVOLVED.

Immigration evaluations are billed at a rate of $100 per hour prorated for actual time spent (except for the written report which is billed separately) and are typically not reimbursable by insurance. A typical immigration evaluation includes a detailed interview with the examinee, administration of at least 2 test instruments, review of written documentation provided by the legal team or other providers of care, and phone conversations or meetings with other providers or individuals with whom the examinee has interacted. These could include medical doctors, other mental health providers, roommates, family members, etc.

A typical immigration evaluation, without collateral interviews needed, can take a minimum of 4 to 9 hours. The time involved/actual cost depends upon amount of documentation to review, interviews/testing being more difficult and taking more time due to the need for an interpreter to be involved, etc.

New evaluation reports are billed at a flat fee of $300. They are detailed and can be as lengthy as 10-12 pages.

Updated assessments are billed at the $100 per hour prorated fee. Total cost will depend on whether or not check-ins with collateral interviewees are needed and the amount of additional documentation to review. At a minimum, they will require a 1-2-hour meeting with the examinee to assess current level of functioning. Again, reports are billed separately; an updated report is billed at $150.

Fees are due as follows:

–Fee for clinical interview is due at the completion of the interview

–Remainder of the evaluation fee is due at the end of the final testing session with the examinee

–Report fee is due when the report is finalized. The report will not be sent to the attorney until the fee has been paid.

If testimony is required, the fee is $200 per hour for the following expenses: preparation time spent with legal team, travel time to and from testimony, wait time at Immigration Court before testimony occurs, and actual time spent in testimony.

Pre-Adoption Assessment Package

Fee for a couple is $800 (if no projective test required). If a projective test is required, the fee is $1600 because I will administer the test to each partner individually, score/interpret each test, and write up the results for each.

Fee for single adult is $400 (if no projective test is required) or $800 if a projective test is required).

1/2 of the fee is due at the end of the first session and the final 1/2 is due when the report is completed. The report will not be sent out until final payment is received.

Testing requirements and subject matter to be covered during the assessment are set by the country from which you are adopting. Please fax to me (at 952-925-5614) in advance of your first appointment the paperwork given to you from the adoption agency or host country outlining the testing requirements. General Psychological Testing – Emotional, Behavioral, Personality Functioning

Often this type of testing is used to clarify a diagnosis or to assist in developing an appropriate treatment plan for therapy. Clients who come for this testing are usually referred by a therapist, social worker, physician, or other professional. Typically such an assessment consists of a 90-minute clinical interview, at least 2 psychological test instruments – some of which may need to be administered by me – my interpretation of test results, and a face-to-face feedback session. Costs of the assessment are based upon the following fee schedule:

90-minute clinical interview $225

Test administration/scoring $175/hour (fee only occurs if a test must be administered by me)

Test evaluation services* $90/30-min. intervals

*Includes interpretation/integration of test results & clinical data, clinical decision-making, treatment planning, report writing, and providing feedback to client

The fee for the clinical interview ($225) will be due at the end of the clinical interview and the remainder of the fee will be due at the beginning of the feedback session. If a report is to be provided to another provider, such as a social work case manager, a therapist, etc., it will not be sent until the full fee has been paid.

Fees for Services Provided

Insurance & Payment Issues

I do not file for insurance reimbursement. I’m considered an “out-of-network provider.” If you would like to use your insurance, I can provide you with a receipt for services, containing diagnostic codes required by insurance, for you to turn in directly to your insurance company for possible reimbursement. Please contact your insurance provider and ask about self-filing options for “out-of-network providers.” See the handout entitled “Tips for Talking with Your Insurance Company Regarding Possible Reimbursement for ‘Out-of-Network’ Services” located at the Forms link – under Supplemental Reading – on this website. This handout provides recommended questions to ask your insurance company in order to determine what your out-of-pocket cost would be.

Payment

I accept cash, check and all major credit cards as forms of payment. Some HSA/FSA accounts allow for payments out of those funds and I can accept payment from a HSA/FSA card.

Cancellation/No Show Policy

For therapy: If you are unable to attend a therapy session, please make sure you cancel at least 24 hours beforehand. If you cancel less than 24 hours in advance, a $50 late cancellation fee will be billed. If you do not show fo your appointment or only stay for a portion of it, you will be billed for the full session charge. If you arrive late for a therapy session, your session will still need to end on time and the usual fee will be charged.

For psychological evaluation: If you arrive late for an appointment, we may not be able to finish what was scheduled for that day and an additional appointment, at your expense, may need to be scheduled. If you cancel less than 24 hours in advance of your testing appointment or do not show for your appointment, you will be billed an additional missed appointment fee of $50.

Cancelling due to Covid-19: There will be no late fee charged if you cancel with less than 24 hours notice if you do so because, on the day of your appointment, you are experiencing symptoms of a possible covid-19 infection or you have tested positive for the virus. If you are a therapy client and feel well enough to have a session, we will do so by telepsychology. Unfortunately for psychological evaluation, doing it by video ethically is complicated. We could likely do a clinical interview by telepsychology although my preference in an evaluation is to do it in person. The testing process will not be able to be conducted online. While there will not be a late fee assessed if you cancel due to Covid-19, if there are too many cancellations of this nature, we will need to discuss how to solve the problem of the absences and services may have to be at least temporarily terminated.
